Misprinting of lab results in Minneapolis VA Health Care System

Veterans Health Administration, LogoMINNEAPOLIS – The Minneapolis VA Health Care System has announced a misprinting of lab results may have impacted as many as 500 Veterans in the Minneapolis VA Health Care System in May 2025.

A news release said “On May 3, 2025, 1,130 patient lab result letters written directly through an automated patient record system failed to print due to a failed IT server.   In pursuing a secondary solution to get patients their lab results, some Veteran information, including full name, address, and lab results were unintentionally printed on the back of another Veteran’s letter.”

As many as 500 Veterans may not have received their first results letter.

To ensure all Veterans received their lab results, all letters were reprinted and mailed on May 29, 2025.

The VA says Minneapolis VA Medical Center Director Pat Kelly has sent a letter to all potentially impacted Veterans.

To prevent this from happening again, the VA decommissioned the printer which had the error, and increased quality control checks in the letter processing area.

The VA says there is no indication at this time that Veteran information has been misused.
